Most programs build a Person-Centered Plan and revisit it annually. At The H.U.B., person-driven is not a document. It is a daily practice. Each resident wakes up with a plan that belongs to them — not a schedule assigned to them. Every morning is intentional. Every choice is honored. Every day is theirs to direct.

Our dedicated Targeted Case Management team walks alongside each resident through every dimension of their life plan — ensuring their Individualized Service Plan (ISP) and Person-Centered Plan (PCP) are not just written with them, but lived with them. Goals are set by the resident. Timelines are shaped by the resident. Progress belongs to the resident. And in alignment with the CMS HCBS Final Rule, every aspect of our model is designed to protect and expand each resident’s rights, autonomy, and full community participation — not as a compliance requirement, but as a non-negotiable commitment.

The TCM serves as the critical bridge between the resident and the full ecosystem of supports surrounding them — the Community Developmental Disability Organization (CDDO), KDADS, medical providers, and community partners — so that no resident ever navigates a complex system alone. But beyond coordination, our TCM is a relationship. A consistent presence. Someone who knows you, hears you, and ensures that every service, every goal, and every day feels the way it is supposed to feel — intentional, chosen, and yours.”
